Discomfort or pain during the labour of any species are caused by uterine contractions and are therefore not governed by the size of the baby, so even though a kangaroo joey is very small at birth the uterus must still expel it by contracting.

Probably not as severe as humans, but it is difficult to tell how much a prey animal feels because it has to mask pain so as not to become a target for predators.
Kangaroos seem to have a quicker labour than humans.

I carry her in a pouch (Baby Bjorn).

it´s a wonder of the Nature,this little joey climp in the pouch,i read from them,now i have see the first Kangaroo birth of my life. it´s veery interesting and very nice. i lost my words...
