its a simple bug, doesnt work any more :P
you use a blazing skin(reflect dmg) on a low lvl char, then berserker spirit and hex to lower his p. def, so ember does a lot of dmg and a lot of dmg is reflected :P
(and propably a bug with dance of medusa, cause when it lands you cant get dmged)
