This is a good song.
To Joan: "St. Theresa" was a crack addict woman that she knew. The woman was prone to hallucinations, but had a sweet personality when not high.
To everyone else: "St. Theresa" is THE actual St. Theresa.
....as such, the video has drug/hallucination references (goldfish and heartbeat strawberry) and also religious references (the real Saint was claimed to be able to levitate, and also felt that sheep were holy).

....with drug symbolism references (e.g., heartbeat strawberry, goldfish, body turning into plants, etc) to Joan's drug addicted "friend" St.Theresa.
When you look at it from that perspective, it makes perfect sense. My favorite one is:
"Every STONE (i.e., crack rock) a story, like a Rosary"... perfect combo of symbolism. :)
The scene is not CGI, it's plastic and makeup.
Joan refused to sit in for the hours it took to put more and more of that on her skin (while taking pictures between each interval)... so they instead did her sister's hair up like her, and had Sarah stand in.
